HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
1. Our pledge
RoadieForge Health, Inc. is required by the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) to maintain the privacy of your Protected Health Information (“PHI”) and to provide you with notice of our legal duties and privacy practices regarding your PHI.
2. How we use and disclose PHI
- For treatment. Your RoadieForge clinician and any consulting providers may share PHI to coordinate your care.
- For payment. We use PHI to bill and collect payment from you or, where applicable, your insurance.
- For health care operations. Internal quality review, staff training, and audits.
- Business associates. Our pharmacy fulfillment partner, telehealth platform, and other vendors who handle PHI on our behalf — all under Business Associate Agreements that bind them to HIPAA standards.
- As required by law. Public health reporting, mandated abuse reporting, court orders, law enforcement requests, and similar legal obligations.
3. Uses that require your written authorization
- Marketing communications involving payment from a third party.
- Sale of PHI (we do not engage in this).
- Most uses of psychotherapy notes (we do not currently maintain these).
You may revoke any authorization at any time, in writing.
4. Your rights regarding your PHI
- Right to access. You may inspect and obtain a copy of your medical record. Request via the portal or by emailing privacy@roadieforge.com.
- Right to amend. You may ask us to correct PHI you believe is inaccurate.
- Right to an accounting of disclosures. A list of certain disclosures we have made.
- Right to request restrictions. You may ask us to limit uses or disclosures for treatment, payment, or operations. We will consider but are not always required to agree.
- Right to request confidential communications. You may ask us to contact you in a specific way or at a specific location.
- Right to a paper copy. You may obtain a paper copy of this Notice on request.
- Right to be notified of a breach. We will notify you if there is a breach of unsecured PHI affecting you.
5. Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at privacy@roadieforge.com, or with the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr. We will not retaliate against you for filing a complaint.
6. Changes to this notice
We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have about you as well as any information we receive in the future. We will post the current Notice in our portal and on this page.
