Informed Consent for Telehealth

Before receiving care through RoadieForge, please read and acknowledge the following information about telehealth services. You may withdraw consent at any time by emailing privacy@roadieforge.com.

1. What is telehealth?

Telehealth involves the use of secure electronic communications (web portal, messaging, phone, and video) to enable a licensed clinician to diagnose, consult, treat, transfer medical data, educate, and provide care using interactive technology between you and the clinician.

2. Nature of telehealth care

  • Your clinical evaluation and any treatment may take place entirely through secure messaging and your intake forms, supplemented by video where indicated.
  • You may be asked to upload photographs, lab results, or wearable data to complete your evaluation.
  • Your clinician may decide, in their professional judgment, that telehealth is not appropriate for your condition and may refer you to in-person care.

3. Expected benefits

  • More convenient access to care without travel.
  • Reduced waiting time.
  • Continuous coordination across your assigned provider, pharmacy partner, and (with your consent) wearable-derived data.

4. Possible risks & limitations

  • Technology issues: connectivity failures can delay or interrupt care.
  • Limited physical exam: certain conditions may require an in-person physical exam or laboratory diagnostics that we cannot perform remotely.
  • Privacy: while we use industry-standard encryption, no online communication is 100% secure.
  • Emergencies: telehealth is not appropriate for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency department.

5. Records

Records of telehealth visits are stored as part of your medical chart. They are subject to the same protections as in-person visits under HIPAA and applicable state law.

6. Costs

Subscription and per-visit charges are presented at checkout. Some products are billed by our pharmacy partner directly; in those cases your invoice will reflect the partner's charge.

7. Patient rights

  • You may withdraw this consent at any time.
  • You may refuse telehealth and request an in-person referral.
  • You may request a copy of any record made during a telehealth encounter.

8. Acknowledgment

By using the RoadieForge service, you acknowledge that you have read this Consent, that your questions have been answered to your satisfaction, and that you agree to receive telehealth services under these conditions.

9. Emergency reminder

If you are experiencing a medical emergency, call 911 immediately. Telehealth is not a substitute for emergency care.
